How would you do usernames?

Imagine you were creating a game expect to be as big as, say, World of Warcraft. However, unlike World of Warcraft you didn't want to make each server independent and want to allow players to have friends lists/communicate across the servers - like Runescape.
Unfortunately, that puts more pressure on usernames! In WoW, a name is unique only to the server - Psychoboy could be on every server. As the created you've decided you only want alphabetic characters - no numbers - in the usernames, but how do you compensate for the demand?
My first idea was titles, Richard the Brave, Richard the Second, etc. Or unique IDs, Richard000. But I wanted to hear your ideas! I don't really like either of mine, the ID defies the point of no numbers and the title removes the fun of custom ingame titles.

Why no numbers?
Even without numbers... 26^10= ~141,000,000,000,000
Even if we assume that .01% of those are pronounceable we still have more than one distinct name for every inhabitant of this current planet.

hey! there is a chance. There are so many "Dragon78114" variants in the game. There are RS usernames like Dragon78112, Dragon78118. I mean it can happen, despite the numerically small chance.

I gave DuskCurse his username. His was once Guthan and he asked me about the naming conventions of human hunters. I explained to him that their names are determined by which combat styles they used, lifestyle choices or acts of fate.
He wanted his name to be of a werewolf who followed Zaros and liked using ancient spells. Dusk came from the Zaros aspect (prior to a lot of other work, Zaros was of dusk and Armadyl was of dawn) and the curse came from the ancient spells (which are commonly referred to as curses due to their aggressive nature).
Anyone else want me to come up with one for them?

Ya... If you could fine a sweet Portuguese word, that is also a chess opening move, on 3 letters (nice and short), it would be awesome if it could start with R.... Ooh and if it could end on -uy, all would be apples
